0de9c41ff4 pulse: introduce pulseaudio input 
It currently use the simple api and is using the latency information
provided only to offset the stream start.

8621a37d9f In video4linux2, in the case the timebase value in ap is 0/0, read the 
timebase value already set in the driver, and set it back in the codec
stream, rather than leaving the invalid value of 0/0.

In particular, fix ffmpeg grabbing timestamps when the timebase value
is not set through the CLI.

987903826b Globally rename the header inclusion guard names. 
Consistently apply this rule: the guard name is obtained from the
filename by stripping the leading "lib", converting '/' and '.'  to
'_' and uppercasing the resulting name. Guard names in the root
directory have to be prefixed by "FFMPEG_".
