f1f6d3615f avcodec: add support for planar signed 8-bit PCM. 
It is found in some 8svx files (e.g. ones created by SoX).
Currently the decoder reuses the 8svx functions because we already have
handling of a single large planar packet for the compressed 8svx codecs.

1993c6849c 8svx/iff: fix decoding of compressed stereo 8svx files. 
Make the iff demuxer send the whole audio chunk to the decoder as a
single packet and move stereo interleaving from the iff demuxer to the
decoder.

Based on a patch by Stefano Sabatini.

b7effd4e83 avio: avio_ prefixes for get_* functions 
In the name of consistency:
get_byte           -> avio_r8
get_<type>         -> avio_r<type>
get_buffer         -> avio_read

get_partial_buffer will be made private later

get_strz is left out becase I want to change it later to return
something useful.

c6610a216e Prefix all _demuxer, _muxer, _protocol from libavformat and libavdevice. 
This also lists the objects from those two libraries as internal (by adding
the ff_ prefix) so that they can then be hidden via linker scripts.

2d162e3825 Make ff_cmap_read_palette static to libavcodec/iff.c. Delete iff.h. 
The iff.h header only declared one function that is now static, the
libavformat/iff.c source file wasn't using it before. Drop the file
entirely.

46dcabf82b Make the IFF demuxer a little more standards-compliant, e.g. respect the size 
fields of common media header chunks (these can have different sizes depending
on the type of IFF file you read), better handle odd sizes (like RIFF, every
field is padded to word) and handle headerchunks after the BODY chunk.

bad5537e2c Use full internal pathname in doxygen @file directives. 
Otherwise doxygen complains about ambiguous filenames when files exist
under the same name in different subdirectories.

dd1c8f3e6e Bump Major version, this commit is almost just renaming bits_per_sample to 
bits_per_coded_sample but that cannot be done seperately.

Also reset the minor version and fix the forgotton change to libfaad.
Note: The API/ABI should not be considered stable yet, there still may
be a change done here or there if some developer has some cleanup ideas and
patches!
