0e69c04773 ogg/speex: set correct timestamp and duration for the first packet. 
The first timestamp should be negative due to delay.
Also, do not set AVCodecContext.frame_size unnecessarily.

15299b3821 Calculate correct packet durations when demuxing Ogg/Speex. This involves 
determining if there is any delay in the first packet and/or any truncation in
the final packet.

eb5f3c5434 Modify the Ogg/Speex demuxer and the libspeex decoder so that they always treat 
a packet of Speex frames as a single frame.
