Revert "all: Don't set AVClass.item_name to its default value"
...
Some callers assume that item_name is always set, so this may be
considered an API break.

lavf/bluray: translate a read of 0 to EOF
...
Yet another case of forgotten 0 =! EOF translation. The libbluray
documentation specifically mentions that a read of 0 is EOF.
